It applies to slwc.life, our booking pages, email and SMS communications, and our in-clinic operations. It also explains how we use the Unified Practice Patient Portal (UP) for health records.

Our Systems and Scope

• Unified Practice Patient Portal (UP): Our HIPAA compliant electronic health record used for intake forms, clinical charting, and any protected health information. All clinical data lives in UP.
• Website and Go High Level: Used for website hosting, online booking, reminders, and SMS or email communications. We do not use Go High Level or standard SMS or email to store or transmit PHI. For sensitive information use the UP portal or speak with your practitioner in session.
• Payments: Stripe processes payments. We do not store full card numbers.

Information We Collect

• Contact and identifiers: name, email, phone, address, preferred name and pronouns if provided.
• Appointment and account data: bookings, reschedules, attendance, communications about scheduling and client support.
• Intake and clinical information (PHI): concerns, goals, history, exam findings, and clinical notes you share during care. Collected and stored only in UP in accordance with privacy law.
• Payment information: card brand, last four digits, transaction IDs. Stored by our payment processor.
• Technical data: device and browser data, IP address, cookies, analytics, and interaction data with our site, emails, and SMS.

Sources: you, automated technologies such as cookies and analytics, and service providers that support scheduling, messaging, payments, and hosting.

How We Use Information

• To deliver services such as scheduling, reminders, check-ins, and client support

• California privacy: we do not sell personal data. California residents may email care@mail.slwc.life with the subject line California Request to exercise CPRA rights.

SMS Program

By providing your number and opting in you agree to receive SMS from SLWC about appointments, service updates, check-ins, educational content, and occasional offers. Message and data rates may apply. Message frequency varies.

• Opt in methods: checking a consent box on a booking form, signing our in-clinic consent in UP, texting our keyword such as RESET, or replying YES to our confirmation text.
• Opt out and help: reply STOP to end SMS at any time. Reply HELP for help or contact information. You may also email care@mail.slwc.life to manage preferences.
• Health information by SMS: standard SMS and email are not secure for PHI. Do not send medical or sensitive details by SMS or email. Use the UP portal or speak with your practitioner.
• Data use in SMS: your phone number and message content are used only to deliver and manage the SMS program. We do not share SMS consent or message content with third parties for their own marketing. Mobile carriers are not liable for delayed or undelivered messages.
• Age: the SMS program is for individuals 18 and older. Parents or guardians may manage communications for minors in care.

Children's Information

Our site is not directed to children under 13. If you believe a child provided personal information without consent contact care@mail.slwc.life.

Security and Retention

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the data. No system is perfectly secure. We retain data only as long as needed for services, legal obligations, or legitimate business purposes. Health records are retained in accordance with North Carolina law and professional standards.
